Below you will find the required forms necessary for all new clients to fill out before your appointment. Returning clients do not need to fill out another round of forms every time. These forms will also be sent to you after you schedule your first appointment, along with your appointment confirmation. In the event that they didn't come to you as planned, they are here for your convenience. The "Liability Waiver" form also contains a "Parental Consent" section, if necessary, and as required for clients under the age of 18.
